Veteran Record Review & Medical Opinion Letters

Case-by-case review

The first step is reviewing whether the available medical records, clinical history, and current concerns provide enough information for a meaningful clinical opinion. In some cases, additional documentation or evaluation may be needed.

Medical opinion letters

If clinically appropriate, a medical opinion letter may discuss relevant medical history, current findings, and the provider’s clinical opinion based on the information available. The content and conclusion of any letter are based on independent clinical judgment, not on a requested outcome.

Separate from disability exams

This service is separate from contracted Veteran Disability Evaluations. Contracted evaluations are scheduled through authorized exam vendors. Private medical record review or opinion letter requests are handled separately and are not part of a contracted exam assignment.

Before scheduling

True North Health Clinic may review veteran medical records and clinical history on a case-by-case basis. Some patients may refer to this type of request as a nexus letter, but a letter is never guaranteed. Any opinion, if issued, is based on independent clinical judgment and available records. The clinic does not act as a veterans service organization, claims representative, attorney, or VA benefits advocate, and does not guarantee VA disability ratings, claim approval, benefit eligibility, or any legal outcome.
