No patient-provider relationship from website use

Visiting this website, clicking a link, sending an email, calling, texting, or requesting information does not automatically create a patient-provider relationship with True North Health Clinic or Joselyn Chai, AGACNP-BC, ARNP. A clinical relationship, if appropriate, depends on scheduling, intake, consent, eligibility, referral or authorization requirements, and clinical acceptance.

Appointment-only clinic

True North Health Clinic is an appointment-only clinic. Patients and visitors should not arrive without a confirmed appointment. For time-sensitive medical concerns, urgent symptoms, or emergencies, please call 911 or go to the nearest emergency department.

Email and text communication

Email and text messaging may not be fully secure. Please avoid sending highly sensitive medical records, claim documents, identification documents, or private health information by unsecured email or text unless instructed and you understand the risks. Email and text are best used for general scheduling, coordination, and non-urgent communication.

Records, forms, and authorizations

Medical records, billing records, forms, reports, or documentation requests may require proper identification, signed authorization, payer instructions, contractor instructions, or other required documentation before they can be reviewed, released, completed, or discussed.

VA, TRICARE, L&I, and DOT / CDL notices

True North Health Clinic is not affiliated with, endorsed by, or operated by the U.S. Department of Veterans Affairs, TRICARE, FMCSA, or Washington State Department of Labor & Industries. Eligibility, referral requirements, authorizations, payment, certification, benefits, ratings, claim outcomes, and coverage decisions are determined by the applicable agency, payer, contractor, examiner, or plan.

Veteran disability evaluations

Contracted veteran disability evaluation appointments, when assigned, are separate from regular treatment visits, VA CCN care, and private medical opinion letter requests. True North Health Clinic does not determine VA disability ratings, benefits eligibility, or claim outcomes. Patients should follow instructions from the authorized scheduling vendor or contractor.

Veteran record review and medical opinion letters

Veteran record review or medical opinion letter requests are considered case by case. Some patients may refer to these requests as nexus letters, but a letter is never guaranteed. Any opinion, if issued, is based on independent clinical judgment and available records. True North Health Clinic does not act as a veterans service organization, claims representative, attorney, or VA benefits advocate, and does not guarantee any VA rating, claim approval, benefit eligibility, or legal outcome.

DOT / CDL medical exams

DOT / CDL medical certification exams, when available, are performed under FMCSA standards by a certified medical examiner. A completed exam does not guarantee medical certification, a two-year certificate, or clearance to drive. Additional records, specialist clearance, follow-up, or shorter certification periods may be required depending on the driver’s medical history, examination findings, and applicable standards.

No emergency communication
