Scheduled by authorized vendors
Veteran disability evaluations are typically scheduled through a contracted exam vendor or authorized scheduling organization. Patients should follow the scheduling vendor’s instructions for appointment details, required forms, and visit expectations.
What to bring
Patients should bring a valid photo ID, appointment confirmation instructions, and any documents specifically requested by the scheduling vendor. If the vendor has provided forms or special instructions, those should be followed carefully.
Evaluation, not treatment
A disability evaluation is different from an ongoing medical treatment visit. The purpose, documentation, and communication process may be determined by the contracted evaluation process rather than by a regular treatment relationship.
Before scheduling
True North Health Clinic may provide veteran disability evaluation appointments when the visit is assigned and scheduled through an authorized contractor or exam vendor. These appointments are separate from VA CCN treatment visits, private medical opinion letter requests, and regular clinic visits. The clinic does not determine VA disability ratings, benefits eligibility, or claim outcomes. Patients should follow instructions from the authorized scheduling vendor.
